We welcome new enrolments. If you would like to enrol with us please visit us at the Akaroa Health Centre. We are happy to introduce you to our services, check your eligibility to enrol and receive subsidised primary health care services and provide you with an Enrolment Form, our Terms of Trade and a brief Health Questionnaire.

Please bring two forms of identification with you (one type of photo ID e.g. Drivers Licence, 18+ or Student ID card) and a New Zealand or Australian Passport.
